Introduction:

This case was completed by Dr. Raul Costa, a Cosmetic and Restorative dentist, practicing at Wimpole Street Dental Clinic in London. You can see more of his work on his Instagram account @raulcosta_dentistry

Procedure

Step 1: Initial Situation

We had secondary caries associated with a defective composite filling on a lower second premolar, and primary caries on the mesial of the adjacent molar.

Step 2: Decay Removal

Removing the decay of the premolar, highlighting the fact this tooth could not be adequately wedged.

Step 3: Greater Curve Matrix Placement

Greater Curve to the rescue, managing to seal the cervical margin, and at the same time, create the correct emergence profile. Invading the space of the adjacent molar is needed in order not to bend the matrix. Note the use of PTFE to increase the ability to seal the cavity from humidity.

Step 4: Matrix Positioning

Step 5: Trimming the Matrix

Step 6: Restoring the Adjacent Molar

One can now use a regular sectional anatomical matrix on the adjacent tooth, wedged, in order to achieve a good contact point and anatomical interproximal shapes.

Step 7: Final Restoration

Final result. Good, strong contact point and smooth anatomical interproximal shapes.
